Loose bolts in that area cause flange separation and because the output is both forward and reverse on that flange, the slop may have ovaled ALL THE BOLT HOLES. Ive seen it on shaft couplers when improper hardware is used. You're already there, replace the trans output shaft or at least machine the surface flat. If the surface is not true, you're playing with fire and it will happen again. I would not use grade 8 but instead replace all bolts after verifying the threads are clean in All the holes and use red loctite and proper torque. DAMN!
I passed your advice onward to my mechanic friend. He is going to check the other 3 holes tomorrow--the bolts are tight as heck he said. If those 3 holes are good, and the threads are good in the "backout" hole, should we just flatten the banged up part and replace the bolt along with lotsa Loctite on all 4?

If the other holes are messed up, and I need a new coupler flange--can you source me that part? He was going to find the part number first thing in the morning, but was sure his distributor doesn't have individual parts for Walters.

All told, he is very open to advice here so that is good. I like him, he is a decent guy.

I'd just clean it up as best you can and give it a try.

You might not be at the end of this problem though.. a locked up transmission output is going to put a lot of strain on the transmission.

We ended up ordering the new output shaft which comes with seals bolts washers etc. Should be here tomorrow. Hopefully everything is OK in there.
